An opaque retractor in the mouth creates the problem it was meant to solve. It holds the cheek clear, but it also blocks the overhead light that has to reach past it, so the field it exposes sits in the retractor’s own shadow. A transparent body lets that light through, and the tissue behind the retractor stays visible rather than becoming a dark edge to the working area.
They come in twos because the mouth needs holding on both sides at once. Retracting one cheek pulls the lip and the opposite side across the field, so a single retractor tends to shift the problem rather than fix it. A matched pair holds the aperture open symmetrically, which keeps the midline where the surgeon expects it — useful in dental and maxillofacial work, where alignment is judged against that midline.
