A calcaneal osteotomy is only useful if the gap it creates is held open at the right width while the graft goes in — and bone under muscle tension closes the moment nothing holds it. The spreader keeps that gap fixed, which frees both of the surgeon’s hands for measuring the defect, trimming the graft and seating it. Without it, one hand is permanently committed to holding the osteotomy apart.
Holding a set width also makes the graft fit predictable. A tricortical graft is cut to the gap, and if the gap changes between measuring and inserting, the graft is either loose — which fails to hold the correction — or has to be forced, which risks splitting it or fracturing the thin cortical shell of the calcaneus. The 15 × 12 mm tip spreads the load across enough bone surface to avoid crushing the cut edges while that width is maintained.
