A single curve turns an instrument in one direction; an S turns it and then back. That double bend is what lets this retractor drop into a narrow opening and then angle its blade back toward the tissue it has to hold — something a singly curved blade cannot do, because by the time it has reached the depth it is pointing away from the surface it should be against.
The tip is sharp rather than blunt, which makes this a retractor for tissue that has to be held rather than merely pushed. A blunt blade retracts by pressing tissue aside and relies on continuous force; a sharp tip engages the tissue and holds it, so retraction is maintained without an assistant leaning on the handle for the length of the procedure. The trade-off is that it must be placed deliberately — an 8 mm sharp blade concentrates load, and it belongs in fascia or dermis rather than against vessels or nerves.
