Textured implants raise the standard a pocket has to meet. A smooth implant will settle into a pocket that is slightly uneven; a textured surface grips the tissue it touches and holds whatever shape the pocket gives it, so any ridge or under-released area left by the dissection stays visible in the result. The spatulated edge addresses that by releasing tissue along a broad flat front rather than in narrow channels, producing a pocket with uniform margins.
Working through all three approaches — axillary, inframammary and periareolar — from one instrument matters because each meets the pocket from a different direction, and a surgeon rarely uses only one.
