330 mm of working length is a great deal of instrument inside a patient, and it exists because the far margin of a subpectoral pocket is genuinely that far from a submammary or axillary incision. Length here buys access without a larger opening — the alternative to a long dissector is not a shorter one, it is a longer scar.
The paddle shape spreads the working surface into a broad flat blade, so each sweep releases a wide band of tissue and the pocket develops evenly rather than as a set of narrow channels. That matters at this length in particular: a narrow blade this far from the hand is hard to keep in one plane, while a paddle rides flat within the plane it was placed in. The gold-plated handle gives a grip surface that stays identifiable at a glance on a crowded back table, useful for an instrument that is picked up and set down repeatedly through a procedure.
