Suction tubing has a will of its own. It is heavy, it is stiff, and it hangs off the side of the operating field — on a fixed connector every one of those forces is transmitted straight into the instrument, rotating the blade in the surgeon’s fingers just as the plane is being followed. A rotatable connector decouples the two: the tube can hang, drag or be repositioned by an assistant while the blade holds whatever angle the surgeon set.
Combining suction with elevation solves the other problem in subperichondrial dissection, which is that the plane fills with blood the moment it is opened and the flap edge disappears. An elevator that clears the field as it works keeps that edge visible continuously, instead of alternating between dissecting blind and stopping to suction. The tip is rounded so that the instrument doing all this cannot score the cartilage it is being drawn across.
