Tunnelling is the technique this elevator is shaped for. Rather than incising down to a plane, the curved tip is introduced through a small opening and advanced beneath the soft tissue, separating it as it goes and leaving the surface above intact. The result is a pocket reached through an entry point far smaller than the pocket itself — which is the whole basis of closed and limited-incision facial work.
The 4.5 mm blade width does the lifting once the tip is under. A narrower blade would separate a thin channel that has to be repeated in overlapping passes, leaving ridges of undivided tissue between them; a blade of this width raises a broad front in one advance, so the pocket is uniform and the tissue above it lies flat afterwards. At 21.5 cm the elevator reaches well beyond its entry point, which is exactly what a tunnelling instrument has to do.
