The palm-held spring handle is what separates this from the finger-held splinter patterns. Extracting a larger object takes sustained grip and steady traction, and a forceps pinched between finger and thumb runs out of force long before the object moves. A spring handle closed in the palm brings the whole hand to bear, and the spring holds the closure so the surgeon pulls rather than squeezes and pulls at once.
Triangular serrated tips supply the other half. A round or flat tip on an irregular object contacts it at one point and rotates off under traction; three faces meeting serrated surfaces engage an irregular shape from several directions at once. The point still lets the tips be worked down alongside an object that is partly buried before the grip is taken.
