This clamp is built around one operation. In Müller’s muscle–conjunctival resection for ptosis, a measured amount of tissue is marked, drawn up, clamped and removed — and the amount removed determines how far the eyelid rises afterwards. Millimetres decide whether the result is symmetrical with the other eye.
What the clamp has to guarantee is that the tissue it holds is the tissue that was marked. Conjunctiva is slippery and Müller’s muscle is thin, so both can slide within an ordinary clamp between measuring and cutting — and once that happens the measurement means nothing. Capturing both layers together between the tarsal border and the marking suture fixes the resection amount at the moment it is set, which is why a dedicated instrument exists rather than a general clamp being used.
