Fixation is the step that makes everything after it possible. The eye rotates freely, so before any incision the globe has to be anchored in one orientation — and it is anchored by taking hold at the limbus, the tough junction of cornea and sclera, then bracing the forceps against the scleral surface so the grip becomes a fixed point rather than a pull.

Multiple fine teeth are what that brace requires. A single pair would hold the same tissue at one point, and the rotational force of an eye resisting fixation would concentrate entirely there — enough to tear through conjunctiva and sclera at the limbus. Spreading the hold over several teeth divides that force. The 10.5 cm length keeps the instrument compact enough not to block the extensive view of the globe the procedure depends on.
