19 cm with a bayonet shank is a combination built for one situation: fine work at the bottom of a deep narrow approach. The length carries the tips to the working plane, and the bayonet step moves the hand sideways out of the corridor the surgeon is looking down — neither alone would be enough, since a long straight forceps still puts the fingers over the opening.
The tips remain extra-fine despite the extra length, which is the difficult part. A longer shaft amplifies hand movement at the tip, so fine jaws at 19 cm demand more of the surgeon than the same jaws at 12 cm. The instrument is specified this way because there is no alternative: the structures at the end of a deep approach are no larger than those at the surface, and they still have to be dissected between rather than pushed aside.
