A ring tip holds by its rim rather than by a face or a point. Conjunctiva is a thin transparent membrane, and any solid jaw closing on it compresses a patch that then shows as a bruise on the white of the eye; a 1 mm ring contacts only around a tiny circle, so the tissue inside the ring is untouched and the grip comes from the circumference. That is about as little handling as an instrument can do while still holding something.
The 6 mm tying platform behind it means the same instrument places the suture it just positioned the tissue for. Three holes in the flat handle reduce weight and give the fingers a defined grip position — useful under a microscope, where the surgeon cannot glance down to check how the instrument is being held.
