The Gorney gives the surgeon a choice between crushing and cutting without changing instruments, and the integrated guard is what makes that choice. With the guard engaged the jaws crush; released, the cutting jaw is brought into play. During septal work the decision between softening a segment and dividing it is often made only once the cartilage is exposed and its stiffness assessed by hand.
The cutting jaw divides cartilage longitudinally without spreading it — an important distinction, because a jaw that wedges as it cuts levers the two sides apart and can propagate a split beyond the intended line into the dorsal strut. Double-action linkage supplies the closing force that 21.6 cm of single-action leverage could not, and the tungsten carbide insert on the crushing jaw maintains its surface texture under that pressure; a smoothed insert slides across cartilage instead of gripping and morselising it.
