The Cottle–Kazanjian solves the same problem as the Hartmann round-jaw cutter but for bone rather than soft tissue: the cutting edge is sharpened on the inner face of the jaws only, and the lateral outer sides are rounded off. Every surface that contacts mucosa on the way in is smooth; the sharpness is reserved for what sits between the jaws when they close.
That geometry lets the instrument be advanced along a narrow nasal passage or beneath a mucoperiosteal flap without lacerating tissue it passes. A spring between the handles returns the jaws automatically between bites, so repeated cuts along a bony margin can be placed without the surgeon repositioning the grip.
